Sorry this post is redundant.
I downloaded the Orthanc Windows installer from the download site, and found it looked like a 32bit server. Is there an installer for a pre-built Windows 7/8.1 64bit Orthanc server available?

Microsoft does not provide a 64bit version of Visual Studio Express, even for open-source projects. As a consequence, we cannot provide pre-built binaries of Orthanc for Windows 64bit.
However, Orthanc is reported to compile properly using Visual Studio 64bit:
