If you have a VPN connection between the main hospital and the branches, you may either use standard DICOM Query & Retrieve or DicomWEB.
If not, then you should somehow expose the branch Orthancs on the WEB and DicomWEB is your only option.
Of course, you should make sure to use HTTPS, enable authentication and users on the branch orthanc and really make sure that everything is secured correctly.

Hi All,
I’m new to this field and have been researching PACS systems in depth, so my question is how two ORTHANC servers in different locations communicate with each other.
So, for example, the hospital has a main branch and a sub-branch, both of which have an ORTHANC PACS server. So, how can a user or physician at the main branch obtain or retrieve patient studies from the ORTHANC PACS server at a hospital sub-branch?
Is this communication possible? If it is possible, then please explain the answer for me. It would be highly appreciated.
