# Ultrasound Studies Open Very Slowly in OHIF Viewer via Orthanc DICOMweb

Hello Orthanc Team,
We are experiencing a performance issue when opening Ultrasound studies through OHIF Viewer connected to Orthanc via DICOMweb.
Issue:
Ultrasound studies take a considerable amount of time to open in OHIF Viewer. In some cases, users wait several seconds before images begin loading. This affects clinical workflow and user experience.
Observations:
- The issue is mainly noticeable with Ultrasound studies.
- Once loaded, navigation is generally acceptable.
- We would like to understand whether this could be related to:
\* DICOMweb performance
\* PostgreSQL indexing/query performance
\* Large metadata retrieval
\* Orthanc configuration
\* Viewer-side rendering
Could the Orthanc team please advise:
1. Are there recommended optimizations for large Orthanc installations?
2. Are there known performance considerations specific to Ultrasound studies?
Any guidance would be greatly appreciated.

And you should also check the size of the US files. Many of them are 200-300 MB and they just need time to be downloaded …
