PostgreSQL dabatabse too large/slow

Dear,

We have an Orthanc instance running with around 12 TB of DICOM storage. We are concerned about the DB growth. At the moment the DB index has 500 GB.

We noticed that the server is getting slower.

There are posible PostgreSQL optimizations/best practices?
Is normal the size of the index (we never store DICOM images in DB)?

Have you considered running VACUUM? This is necessary for PostgreSQL to compact its database after removals:
